Parents, please make sure that you complete all of the Forms and Documents that are required for the new school year. Please see below for a description of each one, along with a link to complete these.
Some of the forms may be completed online, while some will need to be printed and filled out and returned to school.
Parent Contact Form
This form will be used to populate our mailing list for this year as well as information to help with special projects around the school. This form should be filled out online.
LECS Medical Assistance Form
At the beginning of the school year, each school district shall notify parents of each health care service offered at their student’s school and the option to withhold consent or decline any specific service. Parental consent to a health care service does not waive the parent’s right to access his or her student’s educational or health records or to be notified about a change in his or her student’s services or monitoring as provided by this paragraph.
As we do not have a Clinic or Nurse, the services that we can provide are very minimal. Our staff may provide band-aids, ice packs or other basic needs to students who may experience minor injuries while at school. We may dispense medication if parents provide the proper paperwork prior to the medication being given. Parents have the right to opt their child out of receiving any of this basic care. If you opt-out of this, you will be contacted whenever a child would need basic care and would be required to come in and provide band-aids, ice packs or other basic items.

LECS Parental Agreement Form
This form states that you have reviewed all LECS policies, such as the Volunteer, Uniform and Field Trip Policies. It also allows you to give permission for your child to be a Walker (4th Grade and up) as well as allow your child’s photos to be featured in accordance with LECS policies such as for the yearbook, classroom projects, as well as classroom and school websites.
This form should be filled out online.
OCPS Student Code of Conduct
All families are required to review the OCPS Code of Conduct found on our website and then you must print the linked forms, fill them out and return them to your child’s HR teacher.
